		<description><![CDATA[I rarely see 5 Whys used effectively. Although I read a little about TOC, Current Reality Trees, and Peter Senge&#039;s System Thinking, I am more a fan of more systematic root cause analysis. 

Because I&#039;ve been asked many times why I think that 5 Whys is not effective, I finally wrote an article and posted it at:
